Company Profile

The Tetra Company was founded in Chicago, IL, in a single office on the second floor at 350 N. Clark Street, in 1913 with one product - "The TETRA Bandage" - an all cotton bandage that derived its "stretch" by twisting the threads very tightly. A rubber-reinforced bandage line was added in the 50's and remained the company's only product line for ten years.

In 1967, Tetra began adding additional products, namely rib belts, anti-embolism stockings, denture and specimen cups and so on, to the catalog that we are pleased to present to you today.

Today our product line is as diverse as our customers, which include: Hospitals, Surgeons, Podiatrists, Chiropractors, Departments of Correction, Sports Rehabilitation, Sports Teams, Occupational and Physical Therapists, Massage Therapists, Fire Departments, Industrial Offices, Federal, State, City and County Offices.

Policies, Terms, Conditions & Warranties



> Allow three weeks for delivery on all items marked "special order".
> Prices subject to change without notice.
> "TETRA", "EMBO-GUARD", "TETRA-MAT", INSTA-COLD", "INSTA-HOT", "HOT 'R COLD", "EYZE BAG" 
    and "4" are registered trademarks of TETRA Medical Supply Corporation.
> TETRA office hours are 7:30 am to 4:30 pm, central time, Monday thru Friday. After hour and Holiday calls 
    will be answered by a recorder. Orders placed by recorder will be filled on the next business day.
> If you wish to fax your orders, our FAX machine is available 24 hours a day: (847) 647-9034.

TERMS
> To established accounts, net 30 days, F.O.B. Tetra Medical Supply. A late charge of 1.5% per month on the 
    unpaid balance will be added to overdue accounts.
> Minimum billing amount will be $25.00.
> We are striving to increase our product line with quality items and improve our current products. With this in 
    mind, we reserve the right to change, modify, add or discontinue products without prior notification. We will 
    do our best to inform customers using these products in advance, but accept no responsibility for not doing so.

SAMPLE POLICY
> If you wish to order one or more of our products for evaluation, you will be billed at the best quantity price. 
    If the sample is not satisfactory, and it has not been used, you may return it to us and your account will be 
    credited in full. Please let us know when your order is for evaluation to avoid confusion.

EVALUATION
> After 30 days, products purchased for evaluation may not be returned for refund.

RETURN POLICY
> In accordance with our desire to provide the finest quality and service to health care institutions, we will 
    promptly and unhesitatingly, at no cost to the customer, accept for return:
          1. Products which may be defective.
          2. The results of shipping errors on our part.
          3. Products purchased for the first time on a trial basis.
> Within 30 days of shipment, we will accept, subject to a 10% restocking charge, overshipments resulting 
    from a customer error in ordering. Such overshipments must be returned prepaid, and must be in a 
    resalable condition.
> Special conditions may warrant our acceptance of a return after 30 days. Only preauthorized merchandise 
    will be accepted and a 25% restocking charge will be imposed. Customer must return items prepaid. 
    We will not accept any returns after 180 days.
> ALL returns must have a prior written authorization from TETRA Medical Supply.
   Sterile products will not be accepted for return.

TETRA NATAL-MAT
> The TETRA NATAL-MAT is sold as a disposable product intended for a single patient use. 
    This product should be a patient charge item to be disposed of after initial use. We advise that
    you do not send this product home with the patient. Due to the nature of this product, it should 
    be used only under strict and constant medical supervision.
> The TETRA NATAL-MAT is guaranteed for 90 days from date of purchase from TETRA 
    Medical Supply. During this period we will replace any mat that fails, due to manufacturing
    defects, free of charge. Mats that fail due to abuse or mistreatment such as punctures or 
    slashes from pins, needles, scissors, etc., will not be replaced. We do offer a repair kit 
    for these instances, part number 3465-06.
